The Home Health Nurse Residency program is a post NCLEX position tailored for newly graduated nurses from an accredited institution to complete a twelve month structured program designed to transition the new graduate nurse successfully to the role of a proficient Home Health RN Case Manager. The Home Health Nurse Resident will have direct oversight by the Home Health RN Nurse Mentor to ensure that all assignments and responsibilities are competently completed. The Nurse Resident will shadow and be exposed to the various clinical environments and other disciplines working together to create a patients careteam. The program incorporates classroom, virtual learning, and hands-on clinical practice to give the nurse resident a complete understanding of Home Health and how to effectively manage patients in the field. As part of this structured program, the Home Health Nurse Resident will be immersed in an environment to observe the skills and components of being part of the home health team. The Nurse Resident will have the ability to practice selected skills and gain confidence in the homecare environment to prepare for transition into a successful RN case management position that will work independently in the home. The Home Health Nurse Resident will be exposed to a variety of opportunities to perform competency-based skills.
