In this role the Medical Social Worker is responsible for the provision, direction, supervision and evaluation of medical social 
services to clients in the home environment, in accordance with agency policies and procedures, applicable health 
standards, governmental laws and regulations. Proactively meet the needs of and assist the client in the achievement of 
individualized and measurable outcomes.

Schedule

FTE: .10 (PRN)

Hours: 8:00am-4:30pm

Days: Monday - Friday 

Location: St. Augustine, FL

Education

Required: Bachelor's Degree Social Work or Psychology or related area of study

Experience

Required: 1 year Medical Social Work experience

Certificates/Licenses/Registration

Required:

• Motor Vehicle Record (MVR) background check. Must have a good driving record. Valid driver's license and proof of auto insurance required.
• Alzheimer’s Disease and Related Disorder (ADRD) 2-hour course (Home Care Only) required within 90 days of hire for Home Care only.
• Must successfully pass an AHCA Level 2 screening for this position:  https://info.flclearinghouse.com/
