To call discharged Emergency Department patients at home to follow up on their care.

Responsibilities

1. Responsible for using EPIC reports to identify ED patients that were discharged .
2. Responsible for calling discharged patients to follow up on their visit and provide any necessary clarification of discharge instructions.
3. Provides service recovery as needed.
4. Calls to be made within 48 hours of patient discharge.

Qualifications

EDUCATION

Graduate of an accredited school of nursing (Diploma, AD, BS, BSN)

EXPERIENCE

2 years ED experience

LICENSURE

RN In State of CT or Permit; Basic Life Support (BLS)

SPECIAL SKILLS

Computer skills, strong service excellence telephone skills

PHYSICAL DEMAND

May be exposed to infectious diseases. Moderate stress. Light Duty.
