Shift:  Day

Working Hours:  Monday-Friday 8:00 AM -5:00 PM

Summary:

The LPN Care Coordinator works collaboratively with physicians and clinical staff to support patients with chronic conditions and/or complex needs according to guidelines established.

Other information:

1. Current LPN license in good standing (either AR license or multi-state license to include AR), required
2. CPR certification required
3. High School diploma or GED equivalent if working in the lab
4. Three to four years’ experience preferred
5. Ambulatory and/or care management experience preferred
6. Current Driver’s License in good standing and reliable and insured transportation

This job will be authorized 80.00 hours bi-weekly.
