The LPN LTC is responsible for providing specialized care to patients in a long-term care setting. Working under general supervision, this role assesses, plans, and implements interventions to promote optimal healing and prevent complications. This position collaborates with the interdisciplinary team to ensure the delivery of high-quality care and adherence to best practices in wound management.

Essential Job Function
  • Conducts comprehensive assessments of patients' needs to develop individualized care plans.
  • Performs treatments following established protocols practices.
  • Monitors and evaluates the progress of patients, documenting findings accurately and communicating any changes or concerns to the healthcare team.
  • Educates patients, families, and caregivers on proper care techniques, prevention strategies, and signs of complications.
  • Collaborates with physicians, registered nurses, and other healthcare professionals to coordinate care, ensure continuity, and optimize patient outcomes.
  • Maintains accurate and up-to-date documentation of assessments, interventions, and outcomes in the electronic medical record system.
  • Performs other duties as assigned.
Education
  • Required: High School Diploma or equivalent; Diploma in Practical Nursing.
Experience
  • Preferred: 1 year acute care experience
Licensure/Certification/Listing
  • Required: Current LPN license in North Carolina; BLS (CPR)-American Red Cross or AHA Healthcare Provider correct
